We visit and get to know every community whether it is a 55+, Assisted Living, Memory Care, or even Medicaid Skilled Nursing Home. We look for past violations and issues and ensure that every place we recommend is safe and acceptable for your loved one. We work with all budgets and know that no two stories are the same. Because we are a sole proprietor and not a franchise, we can decide what communities we feel are the best options, and do not suggest them based on contract obligations. We only suggest communities that we would feel comfortable putting our own families in. Every placement is free of charge to families as the communities pay us once your loved one moves in. At Graceful Transitions placement should only happen if it is right for your loved one. Safety, dignity, happiness, and quality of life are what’s most important.
